    • 摘要: 目的:心电图和血压异常是判定运动风险最简便而有效的风险指标, 通过对存在运动风险的4049岁人群运动中的心电图、血压以及相关生理生化指标作相关回归分析, 构建4049岁人群运动中心血管风险预测指标体系。方法:4049岁的173名男性与174名女性进行递增负荷测试, 并监测动态心电图与血压变化;将运动中出现心电图ST段下压≥0.1mv或SBP下降>10mmHg作为心电图或血压出现异常的判定标准;通过多元回归和逐步回归分析法分析ST段下压值以及SBP下降值与安静状态下生理、生化指标相关性并建立心电图与血压异常预测公式。结果:1) 运动中男性心电图无异常组ST段下压值与LDH1、LDH1/LDH、OP以及PA存在显著关联, 并构建了预测公式;运动中男性心电图异常ST段下压值分别与HDL-c、OP以及DBP存在显著关联, 并构建了预测公式;2) 运动中女性心电图无异常ST段下压值与BMI、LF/HF存在显著关联, 并构建了预测公式;心电图异常女性ST段下压值与OP、安静心率、CKMB以及AST存在显著关联, 并构建了预测公式;3) 由于血压异常人群数量较低, 因此, 将男性与女性异常者一并进行分析, 运动中血压异常SBP下降值与PWV、PA、LDH1/LDH存在显著关联, 并构建了预测公式。结论:1) 4049岁男性与女性进行运动中容易出现两类风险即心肌缺血与在运动负荷增加时出现血压下降。2) 通过运动激发的4049岁男性与女性心肌缺血与血压异常下降可通过安静时生理、生化指标测试进行预测。

       

      Abstract: Objective:ECG and BP abnormalities in the exercise are the most simple and effective indicators to predicate exercise risk.The aim of the study was to do correlation and regression analysis for the ECG and BP in exercise and the physiological and biochemical indexes in the40 49yr adults who had the exercise risk, then to set up the cardiovascular risk prediction index system for Chinese adults.Methods:173men and 174women who were 4049yr were tested by graded exercise test, the ECG and BP was monitored in test;When ST-segment under pressure≥0.1mV or SBP down>10mmHg during the exercise, the adults had ECG or BP abnormalities.ST-segment under pressure values or SBP down values' relations with quiet physiological and biochemical indicators and the establishment of ECG and blood pressure prediction formula were analyzed by multiple regression analysis and stepwise regression analysis.Results:1) The normal ST-segment under pressure values had significant association with LDH1, LDH1/LDH, OP and PA, also set up the formula to predicate the normal ST-segment under pressure values for adult men in exercise;The abnormal ST-segment under pressure values had significant association with HDL-c, OP and DBP, also set up the formula to predicate the abnormal ST-segment under pressure values for adult men in exercise.2) The normal STsegment under pressure values had significant association with BMI and LF/HF, also set up the formula to predicate the normal ST-segment under pressure values for adult women in exercise;The abnormal ST-segment under pressure values had significant association with OP, Resting HR, CKMB and AST, also set up the formula to predicate the abnormal ST-segment under pressure values for adult women in exercise.3) Men and women abnormal BP had analysis together because of a little people with abnormal BP;The abnormal SBP down values had significant association with PWV, PA and LDH1/LDH, also set up the formula to predicate the abnormal SBP down values for adults.Conclusions:1) 4049yr men and women arise myocardial ischemia and blood pressure drop when load increases during exercise.2) Myocardial ischemia and blood pressure drop when load increases through exercise for 4049yr men and women can be predicted by resting physiological and biochemical tests.
