膝骨关节炎对女性老年人步行动态稳定性的影响
Effects of Knee Osteoarthritis on Dynamic Stability during Walking in Elderly Women

摘要: 研究目的:对比女性老年膝骨关节炎患者与无膝骨关节炎的健康老人行走当中前-后、内-外侧方向上的动态稳定性差异。研究对象:选取性别、身高、体重等身体条件相匹配的女性膝骨关节炎患者及健康老人各10名。研究方法:采用Vicon红外高速运动捕捉系统、Kistler三维测力台系统同步采集两组受试者以日常习惯速度行走当中的运动学和地面反作用力数据。通过动态稳度(Margin of stability,MoS)指标对受试者行走当中的动态稳定性进行量化评定。研究结果:行走的速度、步长、步宽等步态参数,两组间差异不具有显著性(P>0.05)。脚跟触地时刻,两组前-后方向上的MoS差异不具有显著性(P>0.05);内-外侧方向上膝骨关节炎患者组的MoS显著小于健康对照组(P=0.014)。研究结论:在速度相当的条件下,女性老年膝骨关节炎患者与健康老人相比前-后方向上的动态稳定性一致,而内-外侧方向上的稳定性较差,因而在该方向上发生跌倒的风险高于健康老人。Abstract: The purpose of this study was directly quantify walking stability in elderly female patients with knee osteoarthritis(OA)and healthy elder.Ten female patients with knee OA and ten control subjects(matched by height,mass,and gender)walked on the overground at a self-selected comfortable speed.A motion capture system and two force plates were used to determine locomotion.Dynamic stability during walking was investigated by using the margin of stability.There was no significant difference in speed,step length,step width between the patients with knee OA and normal subjects(all P>0.05).Both groups displayed similar anterior-posterior MoS(P>0.05)at foot contact.The patients with knee OA had a significantly reduced medio-lateral MoS compared to normal subjects(P=0.014).Our findings indicate that OA individuals walked with similar stability in anterior-posterior direction,but greater instability and risk of falls in medio-lateral direction compared to normal subjects.
