    Research on the Short-term Characteristics of Plantar Pressure Distribution and Balanceafter Anterior Cruciate Ligament Reconstruction

    • 摘要: 目的:前交叉韧带(ACL)损伤是常见运动损伤之一,探究ACL重建术后患者静态、动态足底压力分布及平衡特征可为临床康复提供理论依据。方法:在探讨性别影响因素的基础上,采集足印,排除足部存在畸形者(扁平足、高弓足),选取ACL重建术后6~8周能正常行走的患者32人(男性21名,女性11名)和健康对照组26人(男性14名,女性12名),采集ACL术后患者和健康对照组足底压力分布和平衡相关参数进行分析。结果:1)ACL重建患者静态足底压力与健康对照组间无显著性差异;2)动态足底压力特征:ACL重建患者外侧足跟面积占全足面积百分比显著降低,与健侧比较,第1跖骨、2-3跖骨面积占全足面积百分比及负荷显著升高,内、外侧足跟负荷明显降低,与对照组比较,外侧足跟负荷明显降低;3)ACL重建组术侧与健侧比较,整足接触阶段(foot flat phase,FFP)时间升高,前足离地阶段(forefoot push off phase,FFPOP)时间降低;与对照组比较,前足接触阶段(forefoot contact phase,FFCP)时间、FFP时间升高,FFPOP时间明显降低;4)ACL重建组术侧椭圆面积明显小于健侧,L/S值明显高于健侧。结论:1)ACL重建术后6~8周患者静态足底压力基本恢复,但动态行走时呈现蹬离不足、整体重心前移步态,术侧最大压力和平均压力均出现明显降低,步行速度减慢,支撑相总时间增长;2)ACL重建术后6~8周单腿站立平衡能力下降,康复过程中应加强下肢本体感觉和肌力训练。

       

      Abstract: Objective: Anterior cruciate ligament(ACL) injury is one of the common sports injuries. The investigation of the characteristics of static and dynamic plantar pressure distribution and balance after ACL reconstruction will provide theoretical references for clinical rehabilitation. Based on the gender difference, the footprints were used to exclude the subjects with foot malformation(flat foot or high arch foot). Methods: 32 patients(21 males and 11 females) who was able to walk at 6~8 weeks after ACL reconstruction were recruited in this study, and 26 normal subjects(14 males and 12 females) without lower limb injury to were set as control group. The parameters of plantar pressure distribution and balance ability were collected from all subjects for analysis.Results: 1) No significant difference was found in static plantar pressure between the ACL reconstruction group and control group.2) The characteristics of dynamic plantar pressure: the percentage of lateral heel area in the affected region was significantly smaller;compared with the healthy region, the area percentage and load of the first metatarsal and 2-3 metatarsal were significantly higher,while the load of the inner and outer heel were significantly lower; compared with control group, the load of lateral heel was decreased significantly. 3) The affected region spent significantly more time in FFP(foot flat phase) but less time in FFPOP(forefoot push off phase) when compared with healthy region; compared with control group, the affected region spent significantly more time in FFCP(forefoot contact phase) and FFP but less time in FFPOP. 4) The affected region had significantly smaller ellipse area, but the L/S value was significantly higher than that of the healthy region. Conclusion: 1) The static plantar pressure was recovered 6~8 weeks after ACL reconstruction, but the dynamic walking showed insufficient pedal, forward movement of the whole center of gravity. The maximum and average pressure of reconstruction side was decreased significantly, and the walking speed slowed down and the total time of support phase was increased. 2) The balance ability of one-leg standing was decreased at 6~8 weeks after ACL reconstruction, so proprioception and muscle strength training of lower limbs should be strengthened during rehabilitation.
