经颅电刺激技术属于兴奋剂吗?

    • 摘要: 经颅电刺激(transcranial electrical stimulation,tES)能够调控脑皮质兴奋性、加强脑网络间连接和同步大脑振荡活动,已在体育科学领域用于提升运动员的运动表现。出于伦理方面的考虑,部分研究人员呼吁世界反兴奋剂机构(World Anti-Doping Agency,WADA)讨论tES技术对竞技体育的潜在影响。通过系统梳理国内外近20年tES技术的相关文献,回顾WADA《世界反兴奋剂条例》中对禁用物质和方法的评定标准,分析tES技术是否符合提高运动能力、危害运动员健康和违背体育精神的“2/3规则”,以探讨其是否属于兴奋剂。研究表明:1)tES能够用于改善人类的运动表现,包括提升肌肉力量、改善耐力表现、增强平衡能力、促进技能习得和认知表现等,但基于运动员群体的研究较少,且集中在经颅直流电刺激技术;2)tES技术是安全的和人体可耐受的,不会对人体健康造成严重不可逆的损伤;3)可穿戴式tES设备是较容易获得的安全技术,且运动员使用此技术时需要结合刻苦训练才能更有效地提升运动表现,与体育精神的核心内涵一致。因此,tES技术不满足“2/3规则”,没有违反《世界反兴奋剂条例》的规定。建议WADA出台相关政策和使用指南,指导教练员和运动员合理使用tES技术。

       

      Abstract: Transcranial electrical stimulation(tES) can modulate cortical excitability, strengthen brain network connectivity and synchronize brain oscillations, and they have been used in sports science to enhance athletes' performance. For ethical considerations,some researchers have appealed to the World Anti-Doping Agency(WADA) to discuss the potential effect of tES on competitive sports. In this paper, we systematically reviewed literatures referring to tES techniques in the past 20 years, and the criteria of substances and methods prohibited by WADA in the World Anti-Doping Code. And we also analyzed whether tES meet the “2/3rule”(performance enhancement, health risk, and violation of “spirit of sport”), to discuss whether it belongs to doping. It is concluded that: 1) tES can be used to improve sports performance in humans, including increasing muscle strength, promoting endurance performance, enhancing balance ability, and facilitating skill acquisition as well as cognitive performance. However, there are limited studies focus on athletes and the tDCS is the major stimulation in previous studies; 2) tES is safe and can be tolerated by humans, and it will not lead to severe and irreversible damage to human health; 3) Wearable tES device is easily accessible, and athletes should combine this technique with hard training to improve performance efficiently, which is consistent with the spirit of sport. Altogether, tES doesn't meet the criteria of 2/3 and doesn't violate the rules of the World Anti-Doping Code. It is suggested that WADA should introduce policies and guidelines for coaches and athletes to apply tES rationally.
